Job Description


The role of the Member Care Coordinator (MCC) is to coordinate and guarantee provision of member services. The MCC supervises a team of direct care workers (DCWs) and reports to the Member Care Supervisor (MCS). The MCC works in conjunction with fellow MCCs.

The MCC is responsible for the scheduling and staffing of services, daily oversight of services, and assisting with the completion and collection of necessary member and DCW paperwork. Working on our frontlines, the MCC is the first point of contact for members and DCWs providing hands on support to both and assuring quality care experiences.  


  1. Performs as a representative of Home Assist Health upholding the company’s Mission, Vision and Purpose.
  2. Staffing and Scheduling of services in accordance to the service/care plan, authorizations, member needs and interests.
  3. Supervises, trains, and mentors a team of Direct Care Workers
    • Coordinates for necessary pre-service and in-service training
    • Conducts supervisory visits
    • Collects, reviews and assures proper completion of timesheets and program data reporting
  4. Assure that Quality Care is being provided:
    • Staff member services/shifts with the best fit DCW in a timely fashion
    • Fill service gaps promptly with an appropriate DCW and when needed arrange for service Orientation.
    • Offer pre-interviews to members, conduct service orientations for first time DCW to member matching and complete necessary paperwork.

Administrative Responsibilities:

    • Documents correspondence logs in accordance with policy
    • Collects and confirms DCW timesheets and related program reports
    • Assures proper authorizations are in place for scheduling
    • Assists as needed with the resolution of incorrect timesheets and cleaning of S-Report
    • Completion and collection of necessary member and direct care worker paperwork as well as distribution of companywide notifications/memos

Program Reporting:

    • Reports Non-provisions of service and related details to MCS
    • Completes Daily Activity Logs
    • Oversight of DCW program reporting for appropriateness, accuracy and completeness (i.e. habilitation data report)

First Point of contact for:

    • Member and Member Representative/s (Family)
    • Direct Care Workers

On rotation, guarantees services with 24/7 on-call support and after-hours coverage. When on-call must answer all calls when unable to answer return voice messages within 15 minutes. Handle all situations with care and a sense of urgency working to resolve situations properly and when necessary elevating issues to MCS. Supervision Received:

Reports to the Member Care Supervisor

Supervision Exercised:

Responsible for providing hiring recommendations, managing all aspects of DCW performance; conducting performance evaluations and addressing performance issues.

Directly Reporting: Direct Care Workers

FLSA Status:                                This position is classified as Exempt under the FLSA.


Knowledge, Skills, and Abilities:

Requires the ability to read, write and speak effectively in English, bi-lingual in Spanish is preferred.  Must possess strong interviewing, counseling, leadership, management, planning and organizational skills.  Requires a basic understanding and knowledge of compliance and safety issues in non-medical home care.  Requires use of personal automobile for making home visits.

Education or equivalency:

High School Diploma or GED equivalency required.  Bachelor’s degree is strongly preferred.


Must have a minimum of four 4 years supervisory/management experience, preferably in a non-skilled home health setting.

Specialized training:

Direct Care Worker or related caregiver training


Requires current CPR & First Aid certification. Be able to pass a comprehensive background clearance. Must have a valid driver’s license that is in good standing and current vehicle insurance and registration.
